New patients should arrive 15 minutes early with insurance identification cards so we may verify patient benefits and complete necessary paperwork. If you would like your complimentary benefits verification done before the first visit, please call (952) 454-1842 at least the day before with your insurance information so that we may have this information ready for you when you arrive.
